fix: add PvFeedFollowLists to Feed Management page, remove activation from Follow Lists 

- Add PvFeedFollowLists component to Feed Management page with section headers
- Remove activation switch from PvFollowListManager (activation is now per-feed on Feed Management page)
- Simplify PvFollowListManager to only handle list creation, editing, deletion

Closes #143
