[ABW]: Collations moved to column definitions

Added root_id to hive_posts
Removed redundant parent_author_id, parent_permlink_id, root_author_id, root_permlink_id from hive_posts and related indexes
Removed obsolete max_cashout_time and allow_replies from hive_posts (still present in view but as constants)
Removed unused (and incorrectly set) is_edited output from process_hive_post_operation
Attempt to fix order in couple of list_comments_by_ functions (failed in case of ordering containing time)
