Preliminary changes to support HAF interface (and data flow) during hivemind massive sync:

- application context is created (if missing)
- implemented application context detaching and attaching
- hivemind is using HAFs app_next_block method to determine range of blocks to be synchronized
- eliminated need to pass a hived API url to perform massive sync
WIP: live sync is not supported yet.
