Add Phase 2: Flatten Include Hierarchy to roadmap

Document the plan to eliminate cascading includes by moving
HAF-specific templates to common-ci-configuration. This will
allow all projects to include directly from common-ci instead
of the current 4-level deep chain (HAF apps → HAF → hive → common-ci).
