issue #153: update documentation about COMMIT after context attach

Althought it was visible in the application alghoritm diagram, it was not
saying in the doc text: COMMIT is mandatory after
'hive.app_context_attach', otherwise hived may remove event which is
choosing by the app to process after attach.
