improve database initialization

1. remove outdated skip reversible blocks mechanism
2. move decision about dump all accounts only to plugin
3. call initialize database once, without any parameters

remove skip_reversible mechanism

skip_reversible blocks mechanism was not used since
all reversible data are removed from hfm when hived connects to it.

better inititialize database procedure
