remove a bunch of unused indexes

This removes some of the indexes suggested by #195 (closed). It only removes the "true hivemind" indexes: several of the unused indexes are created by HAF registration of the tables (and haf should probably not be creating these indexes since this is a non-forking app). But that's a question for another issue (haf#272).
