Improvements of After Initial Sync phase

Removed temporary creation of index hive_votes_post_id_rshares_vote_is_effective_idx (replaced with permanent one covering also block_num) to speedup update_posts_rshares at FAST_SYNC (phase between massive sync and live-sync)
