Slight change in one of indexes defined for hive.operation table

Bartek Wrona requested to merge bw_improved_op_type_id_index into develop

hive.operation has already index covering op_type_id. It would be very useful (for HAF apps) to cover also block_num to improve queries filtering data by both columns. The exact use case is block data processing by HAF Block Explorer application.
